The Company offers two valuable resources, available at no cost to you, to help you navigate the complex world of healthcare: Included Health and TrueCare360.

Who’s Eligible
Both resources will be available to employees enrolled in a Company medical plan through Blue Cross Blue Shield of Massachusetts (BCBSMA) and their family members and they’ll work together seamlessly.
Employees not enrolled in a Company BCBSMA plan and their family members will have access to Included Health navigation services at no cost, but access to network and insurance information will be limited.
Employees enrolled in a Kaiser medical plan: Visit kp.org to learn about additional resources available to you and your family.
Receiving Support
While the support provided by Included Health and TrueCare360 is available at no cost to you, any healthcare these resources recommend or help you schedule will require the cost defined by the medical plan you’re enrolled in. If you’re enrolled in a Company BCBSMA plan, Included Health will let you know what those out-of-pocket costs will be.
Any information you share with Included Health or TureCare360 will be confidential. The Company does not receive your personal information.

Once you set up your account at includedhealth.com/Benefits4Me, you can contact Included Health, 24 hours a day, 7 days a week, by calling 1-833-938-9948, visiting includedhealth.com/Benefits4Me, or on the app available from the App Store and Google Play.

Check Out Community Support!
There are specialized advocates on the Included Health team who focus on LGBTQIA+ and Black community support. They will help you find providers and resources with expertise and experience within these communities. The support they provide spans mental well-being, benefits and policy information, physical health, gender-affirming care, and family-planning resources.

TrueCare360 is a resource through Blue Cross Blue Shield of Massachusetts (BCBSMA), available to employees enrolled in a Company medical plan through BCBSMA and their family members.
Included Health will connect you with TrueCare360 when you need wellness coaching and additional care navigation. BCBSMA and Express Scripts may also connect you to Team Blue.
In addition, a Team Blue Care Manager from TrueCare360 may reach out to you after you receive a diagnosis or when you’re scheduled to receive certain types of care.
